Acromion Osteotomy shoulder dynamic posterior instability

Abstract

Dynamic posterior shoulder instability is a challenging for surgeons. Some options in literature like a glenoid osteotomy, arthrodesis, capsule plication and bone block. The acromion have a important function in posterior shoulder stability. This is a case report of a 24-year-old female patient with recurrent episodes of posterior shoulder dislocation without trauma. The patient experiences recurring pain in the posterior region of the shoulder and limited range of motion. The condition has progressed to glenohumeral osteoarthritis and reduced functionality of the right upper limb due to the inability to elevate the limb because of posterior dislocation. Radiographs of the shoulder, computed tomography and magnetic resonance imaging were used. The patient presented with a hypoplastic glenoid and early-stage glenohumeral osteoarthritis, classified as type B2 by the Walch classification. The technique used by MD PhD Gerber for static posterior instability, involving acromion osteotomy, was our chosen option in this case to restore the function of the upper limb and prevent dislocation. After performing the acromion osteotomy and fixation, releasing the coracoacromial ligament at the acromial part and releasing the pectoralis minor, intraoperatively, stabilization of the glenohumeral joint was verified. The patient has had a 6-month follow-up to date with a functional range of motion, no new dislocation episodes, and complete consolidation of the acromion. Conclusion: Acromion osteotomy proved to be a viable option for patients with dynamic posterior dislocation, who were indicated for arthrodesis or glenoid osteotomy to stabilize the glenohumeral joint, offering a functional, cosmetic, and resolutive treatment method for these cases.
