Application of the concept of uniform loads in complex acetabular arthroplasty using additive technologies

Abstract

Hip arthroplasty in conditions of severe acetabular deformities is a serious problem in modern orthopedics. When analyzing the anatomical features of the acetabulum, we proposed a concept for creating support structures using additive technologies, using the anatomical features of the area of interest. According to this concept, it is proposed to create a reinforcing implant that allows for uniform distribution of the load on all three bones forming the acetabulum and both columns of the acetabulum. Since 2018, 24 operations have been performed on 24 patients according to this concept and 3D printing. The patients were divided into 6 groups:
1. With post-traumatic deformation of the acetabulum
2. With acetabulum fracture
3. With coxitis
4. With periprosthetic infection
5. With aseptic instability of the acetabulum
6. With coxarthrosis with complex anatomy of the acetabulum.
Results: three patients out of 24 ones, dislocation of the endoprosthesis was diagnosed in the postoperative period. The revision surgery with peri-installation of the femoral component was underwent only one of them. In one patient with periprosthetic infection,Klebsiela was diagnosed during surgery using our concept; despite long-term specific antibiotic therapy, it was not possible to cope with the periprosthetic infection. This required the Girdleston operation. Conclusions: 1. The proposed concept allows for reinforcement of both the acetabulum and the pelvic semiring. (Using all three bones forming the acetabulum as a support allows for uniform load distribution). 2. The design allows using its properties as a navigation system. 3. The minimalist dimensions and shape of the support facilitate bone grafting of defects in the area of interest. 4. Limited variability of standard sizes allows offeringit for the serial production.
