Title : Unusual cause of bilateral posterior shoulder fracture dislocation

Abstract

-Background
While the known causes of such an injury is either seizures or electrocution, in this case it was due to trauma which is rare. With such late presentation the management usually is replacement but due to his young age the decision was made to do open reduction and internal fixation for both shoulders. Bilateral posterior shoulder fracture dislocation is rare and not common to encounter in clinical practice. While all the documented cases of bilateral posterior shoulder dislocation in the literature are mainly caused either by seizures or electrocution, our case is caused by trauma to the shoulders.
Case presentation
A 34 years old man who presented to our hospital after two weeks of falling down his bicycle when he was riding downhill and sustained a posterior fracture dislocation of both shoulders. He also suffered a head trauma with no brain injury. decision was made to do open reduction and internal fixation for both shoulders.
Conclusions
What makes this case unique is the presentation of bilateral posterior comminuted shoulder fracture dislocation which is very uncommon to encounter. In the literature there are only few cases with bilateral posterior dislocation and they are caused mainly by seizures and electrocution and not trauma. A young patient, high energy trauma, a very comminuted fracture, first time bilateral posterior dislocation, which was not reduced for 2 weeks and complete rotator cuff tear that can cause long term disability and functional impairment, all these factors made the decision challenging whether to do open ORIF or arthroplasty. After reviewing all aspects of the case and after discussing the options with the patient we chose to do ORIF
