Title: Surgical treatment of limb deformity and disability: a statistical analysis of 36664 cases

Abstract

Aim: Statistical analysis of the single-center large sample limb deformity and disability spectrum, incidence characteristics and treatment strategies, to provide epidemiological basis and guidance for the prevention and treatment of various types of limb deformity and disability. Methods: A total of 36,664 patients were treated from May 25, 1978 to December 31, 2021 from Qin Sihe Orthopaedic Surgery case database. The age, gender, deformity characteristics, composition of disease spectrum, regional distribution and surgical treatment methods of the patients were statistically analyzed. Results There were 21,294 males and 15,370 females in this group. Aged 1-82 years old, the average age was 23.52±17.18 years old, and the proportion of cases aged 6-30 years old was 80.42%. There were 34,795 lower limb surgeries, accounting for 94.90%. The regional distribution covers all provinces in China, regional distribution covers all provinces, municipalities, autonomous regions and 13 foreign countries except Macao. Including neurological, genetic, metabolic, post-traumatic sequelae, congenital, vascular, lymphatic, skin, endocrine, iatrogenic and other 227 etiological diseases, the most 6 types are poliomyelitis sequelae, cerebral palsy, post-traumatic sequelae, spina bifida Sequelae, Congenital Clubfoot and Developmental Dislocation of the Hip. Among the 291 surgical methods, the top 6 are Achilles tendon lengthening, supracondylar osteotomy, calcaneal arthrodesis, tibia and fibula osteotomy, plantar fascia release and peroneal longus muscle replacement of the Achilles tendon. There were 10,235 cases of orthopaedic surgery combined with bone external fixation, including 4,537 cases of Ilizarov ring external fixator and 5,698 cases of combined external fixator. Conclusion: The regional distribution of limb deformity and disability cases in China is wide, and the proportion of male patients is high, mainly in adolescents. The most common causes and diseases are limb deformities caused by neurogenic diseases, post-traumatic sequelae, and congenital and developmental limb deformities. Common surgical methods include soft tissue surgery, osteotomy, arthrodesis, etc., and are often combined with external fixation techniques. Qin Sihe Orthopedic Surgery Database reflects the etiology, disease types, population characteristics and surgical methods of limb deformities that can be treated by orthopedic surgery. It has important reference significance for the development of limb reconstruction surgery in other organizations.

Biography

Dr. Qin is director and professor of Orthopaedic Surgery of the National Rehabilitation Hospital of the Ministry of Civil Affairs of China. By the end of 2021, he had presided over 36664 operations of various limb deformities and disabilities, created the largest database of limb deformity surgery cases with the largest number of diseases in China, and formed Qin Sihe medical concept, diagnosis and treatment style, and limb deformity and disability surgery reconstruction technology system. He has published more than 400 papers and 12 monographs. The lower limb deformations edited by him and published by Springer, has attracted wide attention in the international academic community. He served as the president of the international limb lengthening and reconstruction society (ILLRS) and the international ASAMI society in China; Chairman of China branch of functional reconstruction of limb disability, chairman of China external fixation and limb reconstruction Committee, chairman of "Spina Bifida Society", and chairman of the Organizing Committee of the sixth world limb reconstruction Conference (ASAMI - ILLRS & BR, Beijing 2024).
