Abstract
Massive irreparable rotator cuff tears under 70 are a difficult challenge we have to deal with, because we should avoid a Reverse Total Shoulder (RTS) at that age.
The options we have are tendon transfers, superior capsule reconstructions with autografts, allografts, and synthetic devices/materials, and subacromial spacers, just to “win” some time before we can go to a more definitive procedure (RTS)
I describe pros and cons of those different methods, and show the al arthroscopic technique I use and the results so far ( InSpace balloon, subacromial spacer)
